CONDITION DIAGRAM


The condition diagram (CD) uses a standardized approach to categorizing and diagramming the clinical data, formulating a differential diagnosis, prevention factors, and
interventions (treatment or further diagnostic procedures). It relies heavily on graphic or non-narrative categorization of clinical data and provides students with a concise strategy for summarizing the “universe of the patient’s problems” at a given time. Although currently used in only a limited number of institutions, the graphic method of conceptualizing a patient’s problems is supported by both educational theory and by its proven success with medical students.
